On your initial visit to our office either as a new patient or at time of increased symptomatology you will be asked to fill out required paperwork. Documentation requirements vary from our standard office forms to forms specific to the type of insurance coverage and the insurance carrier with which you have and type of complaint with which you present. These documents are required by your insurance carrier and essential for the submission and processing of your insurance claim.
If you do not wish to fill out the paperwork required by your insurance carrier our office staff will be happy to personally assist you in the completion our standard office forms and accept cash payment for the services we provide.
